Printable medication list

Fill this in once, print it, and take it to every appointment. Free, no sign-up, and it works offline.

Nothing you type here leaves your device. There is no account and no upload — the list is saved only in this browser. You can disconnect from the internet and it will keep working. Clearing your browser data erases it, so print or save a copy when you are done.
Medication Dose When taken What it is for Prescriber
This list is a record you keep, not medical advice. Never change, stop or start a medication on your own — always follow the instructions of the prescriber, and call 911 in an emergency.

What to put on it

The medications most often left off a list are the ones people do not think of as medication: over-the-counter painkillers, vitamins, supplements, eye drops, inhalers, patches and creams. They interact with prescriptions, and a clinician cannot account for what they are not told about.

Write the dose as it appears on the bottle rather than from memory, and bring the bottles themselves to the first appointment after a hospital stay. If two bottles look like the same drug under different names, ask the pharmacist — they will check it in a minute, and it is free.

Keep one copy in a wallet or purse with the insurance cards, and give one to whoever would show up at the hospital if something happened. A list nobody else can find is a list that is not there when it matters.

More on this in our guide to organizing medications after a hospital stay, and what to do when the lists don't match.

If more than one person needs this list. Printing works until something changes and the copies disagree. RelayNest keeps one shared list for the whole family, flags what a hospital started, stopped or changed, and shows who gave which dose. Free, no subscription.

Common questions

Is this really free?

Yes. No sign-up, no payment, no trial. Type the medications, print the sheet, close the tab.

Where is my information stored?

Only in your own browser, on this device. Nothing is sent to RelayNest or anyone else — you can disconnect from the internet and the tool keeps working. Clearing your browser data erases it, so print or save a copy.

Can I share the list with family?

Not from this page, precisely because nothing here leaves your device. Print a copy each, or use the free app, where a family shares one list that updates for everyone.
